PROBLEM SUMMARY:
The CCAF subtask abnormally terminated on the previous
initialization request. CIFI Termination did not recognize this
condition and proceeded to detach subtask.

PROBLEM CONCLUSION:
Clear IFTCB pointer when the CCAF subtask abnormally terminates
during initialization.
