Error description

• This APAR will:
  1) Improve usability of form-based Fast Path reporting by
     providing sample forms.
  2) Provide another form field to allow capture of both abend and
     sync failure codes. The FAILED field will display the latest
     abnormal event code which means that a sync failure is masked
     by a transaction user abend.
  
  Additional issues being fixed:
  1) No LTERM name is being reported in the OVERRIDE LTERM column
     on the Transaction Transit Analysis report. Only c'-       '
     is being printed. This single dash is printed due to a
     problem in the way LTERMOVR is reported for MQ transactions.
  2) ABEND0C4 out of module IPIMAIN CSECT IPIGDISP offset X'3D4'
     when running with the DASHBOARD option. ABENDS0C4 S0C4

  This APAR is changing the following:
  1. Loop in IPI4PRA1 leading to 0C4 in IPIGDISP. Loop in V4TE
  EOD processing of suspended transactions as we wait for the
  transactions to be re-queued.
  2. LTERMOVR not reported for transaction originating in MW.
  Transactions arriving in OTMA via MQ not being checked properly
  for LTERMOVR.
  3. Customers should have sample forms to illustrate how to
  use form-based Fast Path reporting. To improve usability of
  form-based Fast Path reporting, sample forms are needed.
  4. Another form field is needed to allow capture of both
  abend and sync failure codes. The FAILED field displays the
  latest abnormal event code which means that a sync failure
  is masked by a transaction user abend.
