Error description

• QMF can abend with an ABENDS0C4 ABEND0C4 in DSFMCTL when
  multiple forms are used in a QMF PROC if use of a form that
  contains BREAKs is followed by use of a form that contains no
  BREAKs.

  * PROBLEM DESCRIPTION: An ABENDS0C4 ABEND0C4 can occur in      *
  *                      DSQFMCTL if a QMF PROC that contains    *
  *                      more than one FORM is executed.         *

  Some variables that describe FORM characteristics were not being
  reinitialized between uses of different FORMs that are used
  by a QMF PROCEDURE.  A FORM that contained EDIT CODES of
  BREAK and SUM was followed by a FORM that contained no EDIT
  CODES, yet the second FORM was processed with a variable set
  to indicate it contained a BREAK.  That flag allowed code to be
  processed that led to a fetch protection abend.
  

Problem conclusion

• Code was amended in DSQFPREF to reset form flags when a new
  report is being prepared, and a check of form flags that are
  used only in conjunction with EDIT CODEs was added to DSQFMCTL.
