PI67633: Multiple server applications are added to the virtual cluster list

Error description

Rapid restarts of applications and servers can lead to a
situation in which server applications are added to the
server
application list multiple times. When deleted these
references
are only removed once,resulting in invalid pointers
remaining in
the server applications list.

When servers and applications are restarted rapidly it's
possible that a virtual cluster will receive more than one
reference to an existing server application. When this server
application gets deleted it is only removed from the cluster
once. This problem can be discovered by using gdb to examine a
crash of the system and discovering invalid references in the
virtual cluster's appList.

Problem conclusion

This fix ensures that a server application can only be added to
the virtual cluster once in order to maintain the integrity of
that list.

The fix for this APAR is currently targeted for inclusion in fix
pack 8.5.5.11.  Please refer to the Recommended Updates page for
