Error description

* AsyncListener is registered on
WebAppInvocationCollaborator
preInvoke when isAsyncSupported and isAsyncStarted are both
true

* No calls to onStartAsync are ever made to our listener

* If this listener is the only AsyncListener, no methods are
called during async servlet processing

* If another listener is registered in user code, both
listeners are called correctly

Local fix

register an additional listener in user code

When an AsyncListener is registered internally with the
WebContainer's collaborator, its onError method will not get
invoked if it is the only registered AsyncListener.

Problem conclusion

The WebContainer has been modified to trigger the
AsyncListener's onError method correctly.

The fix for this APAR is currently targeted for inclusion in fix
packs 9.0.0.2 and 16.0.0.4.  Please refer to the Recommended
