Error description

• The DB2 Administration Tool for z/OS' ALT function incorrectly
  structures DDL when changes are made to add an index column
  as an INCLUDE.

  * PROBLEM DESCRIPTION: When using ALT to alter a unique        *
  *                      index and add an INCLUDE column, the    *
  *                      job drops and recreates the index,      *
  *                      but the index is not created correctly: *
  *                                                              *
  *                      For example:                            *
  *                      The original index is defined as:       *
  *                                                              *
  *                      CREATE UNIQUE INDEX >index_name<        *
  *                      ON TABLE >table_name<                   *
  *                      (COL1 ..                                *
