Error description

• When upgrading to 5.0.0.2, you will see an exception:
  
  com.urbancode.cm.db.updater.ApplyException: Duplicate key name
  'keyname'
  
  To fix this issue, remove the index or to have the following
  warn instead of error:
  
  CREATE INDEX <keyname> ON
  BUILD_LIFE_LABEL(BUILD_LIFE_ID);
