PH66826: DFHME0111 MESSAGE ISSUED FOR MISSING INSERT 4 FOR DFHMQ0704 MESSAGE

Error description

• In CICS v6.1, some DFHMQ* messages were updated to include the
  name of the MQMONITOR in the message text if an MQMONITOR is
  associated with the CICS-MQ bridge that issued the error for the
  call. If there is not an MQMONITOR associated with the call,
  then a 4th insert may not be passed to the Message domain. In
  this case, DFHMEBU issues a DFHME0111 message:

Problem conclusion

• DFHMQBR0 has been changed to add the MQMonitor name to message
  insert 4 for DFHMQ0704.
