Error description

• CDC can sometimes fail with "Log reader reported an error with
  return code 12 and reason code 0x1230e03". The z/OS job logs
  contain "Failed to attach asynchronous LRD thread." The z/OS
  message does not provide sufficient information to determine the
  cause of the failure.

Problem conclusion

• AttachWaitThread was modified to report different return codes
  based on different failure scenarios. The return codes contain
  information about which step failed and the return code for
  that step. The Db2 log reader traces the return code before
  tracing "Failed to attach asynchronous LRD thread.
