Problem conclusion

• Update x12_skeleton.mtt, compliance_check.mms and
  hipaa_data_compliance.mms to allow the full 18 digits to be
  pushed to the 277CA when total claim amounts in transaction or
  per provider are greated than the 8.2 allowed for other monetary
   amounts.
