Problem conclusion

• To include a valid Element level CTX segment for situation rules
  where the CTX element is not present, but CTX Segment identifier
   is key to understanding the failure, provide location in
  transaction of then known 999 IK3 Segment for the error also in
  the CTX.

  Healthcare Pack HIPAA 10.0.0, 10.0.3, 10.1.0, 10.2.0, 10.2.1
