Error description

• When the OAuth runtime attempts to add the EXTENDEDFIELDS
  column to a database, if the database does not support the
  column, an error like the following will be emitted
  repeatedly.
  Stack Dump = com.ibm.db2.jcc.am.SqlSyntaxErrorException:
  "EXTENDEDFIELDS" is a duplicate name.. SQLCODE=-612,
  SQLSTATE=42711, DRIVER=4.19.77
  Stack Dump = com.ibm.db2.jcc.am.SqlSyntaxErrorException:
  [jcc][t4][20111][11366][4.19.77] The value of a host
  variable is too large for its corresponding use. Host
  variable=9. ERRORCODE=-4461, SQLSTATE=42815
  The error should be emitted only once.

Problem conclusion

• The OAuth runtime is updated to emit the "EXTENDEDFIELDS" is a
  duplicate name error only once.
