Error description

• I have two customers who have done an over the top install of CA
  from 11.0.12 to 11.1.1 and the insertable objects button is
  invisible. If you hover over it you'll see it's there but it has
  no thumbnail, so essentially blank. You'd only know it's there
  if you've been told it is.
