Fault Analyzer Synopsis states:
The system load of module IGZUOPT failed. The load module was
not found in the standard MVS search path.

Investigation found Fault Analyzer to be picking up the second
Control Information Block (CIB) which contains IGZUOPT, instead
of picking up the first CIB which contains the missing user
program name, when formatting it's error message.
