Error description
This is most likely to happen on 32 Bit executable. The problem happens when the attachment name in the $File item is greater than 253 characters, causing the stack to get overlaid. The crash protection code will ban the document after the 3rd crash.

Problem summary
Not common, but if you have an attachment name with more than 253 characters and are running on a 32 bit Domino/Traveler server, the long file name may cause the server to crash when you sync the attachment.
Problem conclusion
The Notes Traveler server was updated to handle names longer than the data stucture could normally hold.
Temporary fix
