Error description

• Steps to Reproduce
  
  Go to the Organizations application
  Bring up your Org.
  Select Action > Inventory Options> Inventory Defaults.
  Make sure that Automatically create usage documents for new
  reservations? is checked.
  Go the Wotrack application.
  Create a new basic work order.
  Populate the Job Plan with JP11220.
  Save the record.
  Click on the Plans tab and change status on Task 10 to approved.
  Take note of the work order number.
  Go to the Inventory Usage application
  In the description field filter for your work order.
  Bring up your inventory usage record.
  Notice that the inventory usage lines is reading 1,4,6,8 all for
  task 10.
  Go back to Wotrack application
  Bring up your work order.
  Click on the Plans tab > Materials sub tab click new row.
  Add 3 more material line items against Task 20 example
  Task 20, item 6I-2499 from the CENTRAL storeroom. Quantity of 1.
  Task 20, item 6I-2500 from the CENTRAL storeroom. Quantity of 1.
  Task 20, item GLOVES  from Central storeroom Quantity 1.
  Save the record.
  Change status on Task 20 to approved.
  You should be presented with the following popup message
  
  BMXAA4593E - Could not change Activity 1202, task 20 status to
  APPR.
  BMXAA4129E - The record for Site=BEDFORD, Usage=XXXX, Line=8
  already exists.
  
  This validation error is thrown when an attempt is made to
  insert a new record with a unique key value that already exists
  in the database. Ensure that the key value for the given record
  is unique
  
  Yes line 8 already exists as it entered lines 1,4,6,8 when we
  approved task 10.
