Error description

* USERS AFFECTED:
* Systems running the VIOS 2.2.4.x, 2.2.5.x, or 2.2.6.x levels.
  **************************************************************
* ERROR DESCRIPTION:
* Failures during thick LU creation can result in errors in the
* pool metadata that can either allow blocks to be mapped by
* multiple LUs, or allow the pool to try and free blocks
* twice.
* These errors can cause the pool to fail to mount or
* potentially cause undetected data loss.
  **************************************************************
* RECOMMENDATION:
* Install APAR IJ04826.
* Prior to fix availability, an interim fix is available from
* either
* ftp://aix.software.ibm.com/aix/ifixes/ij04826/
* https://aix.software.ibm.com/aix/ifixes/ij04826/
* Installation of the ifix requires a reboot.
