Sharing care plans with others

You can download a client's care plan in PDF format to manually share with your client or others. If your organization is subscribed to notifications about Share Care Plan updates, you can send the PDF to an external party, to your client or to a registered care team member who does not have access to Watson Care Manager. The Share Care Plan report includes the demographic, core care team, programs, goals, care team actions, and client actions information. In addition, the client's current medications, allergies, assessments, and conditions are included. You can use the information to assess the progress of the plan and make informed decisions about the client's care.

About this task

You can download or send the default Share Care Plan report template or a different report template, if configured. The options to download or send a Share Care Plan report are displayed only when you select a template name.

A history of all client-related care plan reports sent from Watson Care Manager to an external party is maintained. You can see a complete history of dates and times when a report was sent, the intended recipients, and any notes that were documented relating to the client. This information can help you to determine when an external party was most recently updated about a client and whether it is necessary to send them another report.

Procedure

  1. Sign in to your account.
    Your home page opens.
  2. To see your clients, from the Side Navigation Menu, select Clients > My Clients, or search for other clients.
  3. Click a client name.
    Their Summary View opens.
  4. Select Summary for a snapshot of the most important information about the client.
  5. To generate the Share Care Plan report, select Share Care Plan.
    The Share Care Plan page opens.
  6. Select a report template.
  7. Choose one of the following options:
    • Download the Share Care Plan report.
      1. Select the Download action and then click Download button.
      2. Browse to where you want to store the PDF on your computer, and save the document.
      3. Distribute the care plan to relevant parties.
    • Send the Share Care Plan report to an external party.
      1. Select the Send action.
      2. Optional: From the Share With list, select one or more external parties to receive the Share Care Plan report. The Share With options are configured by your administrator. If no values are configured or the list is configured to be hidden, the Share With list does not display and the Share Care Plan report is sent to a relevant external party as configured by your organization.
      3. Optional: Enter additional notes for the recipient that relate to the reason you are sending them the Share Care Plan report. You can enter a note of up to 2000 characters.
      4. Click Send to send the details that you entered and the Share Care Plan report in PDF format.
        Note: The Send option is only available if your organization is subscribed to notifications about updates to Share Care Plan updates via webhooks.
  8. Select Include Sensitive Notes check box to include any sensitive notes in the PDF. By default, the check box is unselected.
    The Include Sensitive Notes field is only displayed if configured and if you are a granted a security role with access to sensitive notes.
