Adding goals to plans

You can add a goal to the client's plan. You can add a suggested goal, add a goal from the library, or create an ad hoc goal.

About this task

Use existing goals where possible, but if a suitable goal does not exist, you can create an ad hoc goal. Ad hoc goals are specific to a client and are not available for care team members to add for other clients. You can add a goal that was previously added to the plan, if the previous goal was completed and the time frames for the goals do not overlap.

Procedure

  1. Sign in to your account.
    Your home page opens.
  2. To see your clients, from the Side Navigation Menu, select Clients > My Clients, or search for other clients.
  3. Click a client name.
    Their Summary View opens.
  4. Click Plan > Plan.
  5. Choose one of the following steps to add a goal:
    1. Click the Suggested Goals icon under Goals to see all suggested goals and click the Add Suggestion icon to add a suggested goal.
      The add goal window opens.
    2. Click the Add Goal icon under Goals.
      The goal search opens. Choose one of the following options:
      • Type a goal name to find a goal in the library and select a goal. The add goal window opens.
      • To add an ad hoc goal, select the New Goal menu option. The New Goal window opens.
  6. Enter the goal details:
    • Name - For a new goal only, enter the goal name.
    • Programs - Choose a program to associate with the goal. If the client is assigned to one program, that program is selected by default. When the client is enrolled in multiple programs, you can select one or more programs to associate with the goal.
    • Source - If configured, you must select one of the configured values that indicate the source of the goal.
    • Original Source System - If applicable, you can select one of the configured values to indicate where the goal originated.
    • Focus Areas - You can associate the goal with one or more focus areas, which are categories for reporting and evaluation purposes.
    • Type - You can indicate whether the goal is a long-term or short-term goal.
    • Start Date - You can set the start date in the past, present, or future or accept the default value.
    • Target Value - You can enter a value that represents achievement of the goal.
    • Target Date - You can set the target completion date, which must be later than the start date.
    • Stage of Change - You can select a value to indicate the client's readiness or motivation.
    • Confidence - You can indicate the client's confidence about the goal, on a scale of 1 (low) to 10 (high).
    • Importance - You can indicate the importance of the goal to the client, on a scale of 1 (low) to 10 (high).
    • Reason - You can enter a reason for the goal.
  7. Click Save.
    The goal is added to the plan.
  8. Optional: Click any goal name to see the goal details.
