Complete the following steps to capture an assessment score only. Enter a score for each
measure that is assessed to generate the overall assessment score and results.
About this task
If configured to be shown, you can also capture the source of the assessment score and the
name of the system where the assessment score originated. When you capture a score for an assessment
only, the assessment is completed when you save. You can repeat a completed assessment. You cannot
resume completed assessments.
Procedure

Sign in to your account.
Your home page opens.

To see your clients, from the Side Navigation Menu, select
, or search for other clients.

Click a client name.
Their Summary View opens.

Click .

Click New Assessment Score.
The
New Assessment Score page opens.
Tip: When completed
assessments exist for the client, you can select the action in the row of any assessment to run a new instance of the
assessment.

Select an assessment from the list and click Next.
The assessment opens.

Enter the date when the assessment was conducted.
- From the Source menu, if applicable, select one of the configured
values that indicate where the assessment was obtained from.
- From the Original Source System menu, if applicable, select one of
the configured values that indicate where the assessment originated.

For each measure that was assessed, enter the score that was achieved by the client when the
assessment was conducted.

Click Save to generate results and suggestions.
The results are displayed on the page. You can see the suggestions generated for an assessment
on the Plan page.

Click Cancel to close the assessment without saving.

Click Back to return to the previous page.

To see the suggestions:

Go to the Plan page by clicking in the navigation menu.

The suggested goals and actions are displayed on the Plan page.
