Enrolling clients in programs

After a program is assigned to a client, you gather information to decide whether to enroll, or not enroll, the client in the program. Enrollment means that the client has agreed to participate in the program and begin the activities of the care plan.

About this task

After you assign a program to a client, all program questionnaires and assessments become available.

You can use the Care Team and Other Items clusters to quickly remove care team members and their open assigned/unassigned items when updating a client's program status to Not Enrolled. The Care Team and Other Items clusters are conditionally displayed depending on the selection in the New Status field. The Care Team cluster displays when at least one active care team member is on the care team list. The Other Items cluster displays if there is at least one open unassigned active referral or open tasks that are assigned to a role or assigned to other users who are not on the care team, or open care team actions that are assigned to a role or assigned to other users who are not on the care team.

Procedure

  1. Sign in to your account.
    Your home page opens.
  2. To see your clients, from the Side Navigation Menu, select Clients > My Clients, or search for other clients.

    By changing the filters on My Clients page, you can change your client list view. For example you can apply a filter to view only your High Priority clients. Click Add Filter to add a filter and further refine your client list view by Name, Date of Birth, Priority, Programs, or Phone Number. When you filter by Name, you can search by Last Name, First Name to return a client record that exactly matches that name. If you do not know the exact details to search for, you can complete a partial search by entering one or more characters in the client's name. In this case, the search results might return multiple name records that contains the partial string or character that you entered. Special characters are supported in name searching, for example, you can search for the last name of O'Neill. Note that searching for ONeill will return O'Neill name records but a search for O' Neill (with a space) will not return O'Neill name records.

  3. Click a client name.
    Their Summary View opens.
  4. Click Programs > Programs in the navigation menu.
    The Programs list is displayed.
  5. Click Update Status in the Actions menu for a program.
    The Update Program Status dialog box is displayed.
  6. When you update the status of a program, you can select a reason from the configured list of program status change reasons that are specific to the status selected.
    1. Select Enrolled or Not Enrolled .
    2. Enter the date and time of this update.
      The current date and time is entered by default. You can enter a prior date and time if the status change happened earlier.
    3. If you update the status of the program to Not Enrolled, you must select a Reason for not enrolling the client in the program.
    4. If you re-enroll a client in a program which is previously completed or disenrolled, you must select a reason.
    5. If you select Other as a reason, Other Reason is displayed and you must enter a reason.
    6. Add comments, as needed.
  7. When changing a client's status, check the boxes beside one or more care team members' names from the Care Team cluster to remove them from the client's care team.
    When a care team member is selected to be removed, the care team member is removed and -
    • All assigned tasks are closed with the closure reason, Did not enroll in program.
    • All assigned referrals are rejected with the reason, Did not enroll in program.
    • All care team actions are completed with the outcome, Did not enroll in program.
    • The selected care team members will no longer be subscribed to the client's Alerts.
  8. When changing a client's status, check one or more boxes in the Other Items cluster so items such as Unassigned Referrals, Open Tasks and Open Care Team Actions are no longer associated with the client.
  9. Click Save.
  10. Review any tasks that program enrollment has generated for the care team. Complete tasks, as needed.
  11. Optional: Add goals that the program suggests for the care plan. Review the actions that might be associated with such goals.
