Example assessments and questionnaires reports
The following lists some example reports you can create with assessments and questionnaires data.
- A list of completed assessments for clients, by assessment name, date completed, the assessment questions, the clients' answers to questions, the measures assessed, the assessment score, and the user who completed the assessment.
- A list of assessments for each client by assessment status, care manager and program.
- A list of Score Only assessments by assessment measure and score for each client.
- A list of a client's assessment questions and their answers, by date, if the assessment was run multiple times.
- A list of completed questionnaires by client, assessment name, date completed, assessment questions, clients' answers to questions, measures assessed, assessment score, and the user who completed the assessment.
- A list of a client's in progress questionnaires.
- A list of questionnaires for each client by questionnaire status, care manager and program.
- A list of questions asked when a questionnaire was run and their answers, by date, if the questionnaire was run multiple times.
