Individuals data sets
The Individuals folder contains data sets that you use to create reports relating to a client's demographic information, contact details, programs, and care team.
Note: Individual Demographics is the primary data set in the
Individuals folder. You must use data items from this data set when you join
data sets in reports that relate to clients.
The Individuals folder contains the following data sets:
- Care Team
- This data set groups data items that relate to a client's care team members and their role in the care team. If you want to identify the care team member who added the individuals, use this data set with the Users data set in reports. If additional roles are added for a care team member who is already on the client's team, their previous role is end dated and a new instance of that role is created with a new start date.
- Primary Care Team Member
- This data set groups data items that relate to members of the client's care team who are responsible for the client’s care plan and programs.
- Individual Address
- This data set groups data items that relate to a client's address such as the address details, address type, and whether the address is the client's preferred address.
- Individual Address History
- This data set groups data items that relate to a client's address change history such as when and who created or updated the client's address. Use this table with the individual demographics table to identify when changes were made and by who to a client's address details.
- Individual Demographics
- This data set groups data items that relate to client's demographics data such as their name, marital status, and gender. In addition, this data set groups a client's preferred or most recently updated address, phone number, and email address data.
- Individual Tags
- This data set groups data items that relate to a client's tags, such as additional information that describes them like whether they are a member of an organization or group, for example, Student, Football Team.
- Client Status History
- This data set groups data items that relate to status changes for a client, such as the client's status, Active or Inactive, the reason for the client status change, and the date when the status was updated.
- Marital Status History
- This data set groups data items that relate to a client's marital status change history such as when and who created or updated the client's marital status. Use this table with individual demographics table to identify when changes were made and by who to a client's marital status.
- Individual Email
- This data set groups data items that relate to a client's email address such as the email address details, email address type, and whether the email address is the client's preferred email address.
- Individual Email History
- This data set groups data items that relate to a client's email change history such as when and who created or updated the client's email. Use this table with individual demographics table to identify when changes were made and by who to a client's email details.
- Individual Identification
- This data set groups data items that relate to client's identification types such as their Employee ID or passport, and the number associated with the identification type.
- Individual Identification History
- This data set groups data items that relate to a client's individual identification change history such as when and who created or updated the client's individual identification history. Use this table with individual demographics table to identify when changes were made and by who to a client's individual identification history details.
- Individual Phone
- This data set groups data items that relate to a client's phone number such as the phone number, phone type, and whether the phone number is the client's preferred phone number for calls and texts.
- Individual Phone History
- This data set groups data items that relate to a client's phone number change history such as when and who created or updated the client's phone number. Use this table with individual demographics table to identify when changes were made and by who to a client's phone number details.
- Individual Name History
- This data set groups data items that relate to a client's name change history such as when and who created or updated the client's name. Use this table with individual demographics table to identify when changes were made and by who to a client's name.
- Individual Gender History
- This data set groups data items that relate to a client's gender status history such as when and who created or updated the client's gender status. Use this table with individual demographics table to identify when changes were made and by who to a client's gender status history.
- Individual Birth and Death History
- This data set groups data items that relate to a client's birth and death status history such as when and who created or updated the client's birth and death status history. Use this table with individual demographics table to identify when changes were made and by who to a client's birth and death status history details.
- Individuals in Programs
- This data set groups data items that relate to a client’s programs such as the program name, program status, and the date and time from which a program is in a status. You can use this data set with the Individual Demographics data set to report on clients with programs. You can use the Users data set with this data set to report on which users are responsible for a client's programs. For example, a user who is not part of a client's care team can assign a program to a client.
- Programs
- This data set groups data items that relate to a client’s programs such as the program name, program status, and the date and time from which a program is in a status. You can use this data set with the Individual Demographics data set to report on clients with and without programs.
- Users
- This data set groups data items that relate to users who are not in a client's care team, but might have interacted with the client. For example, a user who assigned a program to the client.
- Alerts
- The data set groups data items that relate to a client's alert details such as the alert name, priority, names of care teams members who are notified about the alert, and the care team member who created the alert.
- Alert Comments
- This data set groups data items that relate to comments that are entered for an alert.
- Alert Notifications
- This data set groups data items that relate to who is notified of an alert, such as the alert name and the names of the care team members who are notified.
