Services data model

The Services data model shows how the data sets in the Services folder relate to each other, from the perspective of planned services for clients. The data model determines the report behavior you can expect to see when you use the data sets to create client services reports.

For example, let's suppose you want to report on client services and their associated goals. Services is the primary data set in your report. To create your report, use the Services data set with the Individual Demographics, Services By Goal, and Goals data sets.

Later, suppose you want to expand your report to display a client's programs. Because your report uses the Services and Individual Demographics data sets, use the Individuals in Programs data set with Individual Demographics to display the client's programs.

In the diagram, you see the Individual Demographics data set. This data set represents the client.
Remember: Services and Providers are both primary data sets. When your report is specific to client services, you must use data items from the Services data set. The report displays a single row for each join.

When your report is specific to providers, you must use data items from the Providers data set. For information about the Providers data model, data sets, and relationships, see Providers data model.

  • The identification is associated with one or more services.
  • If a service has no associated individual identifications, the individual identification data items display empty cells.
  • If a service has one or more individual identifications, a single row displays for each identification.
Services ←→ Individual Tags
  • A service is associated with zero or more individual tags.
  • An individual tag is associated with one or more services.
  • If a service is associated with multiple individual tags, a single row displays for each record.
  • If a service has no associated individual tags, the Individual Tags data items display empty cells.

  • A client has zero or more care team members.
  • The care team is associated with one client.
  • A single row displays for each care team member for the client.
  • If a client has no care team members, the Care Team Member data items display empty cells.
Care Team ←→ Primary Care Team Member
  • A care team member is or is not a primary care team member.
  • A single row displays for each primary care team member.
  • If a care team member does not fulfill the primary role on the care team, the Primary Care Team Member data items display empty cells.

  • A client has zero or more programs.
  • A program is associated with one client.
  • If a client is associated with one or more programs, a single row displays for each program.
  • If a client has no associated programs, the Individuals in Programs data items display empty cells.

  • A service has zero or more notes.
  • A note is associated with one service.
  • If a service has multiple notes associated, a single row displays for each note.
  • If a service has no note associated, the note data items display empty cells.

  • A note has zero or more notes.

  • If a note has multiple comments, a single row displays for each comment.
  • If a note has no comments, the comments data items display empty cells.

  • A client service is or is not a provider service.
  • If a service is not delivered by a provider, the Provider Service data items display empty cells.
Providers ←→ Provider Services
  • A provider has zero or more provider services.
  • A provider service is associated with one provider.
  • If a provider is associated with one or more provider services, a single row displays for each provider service.
  • If a provider has no associated provider services, the Provider Services data items display empty cells.
