Use this section of the form to identify the contract type, provider rating, and provider type for the record.
| Item | Description |

| Contract Type | The type of the contract that is associated
with the service provider, such as architectural, asset lease, real
estate lease, and service agreement. Click the Search icon  and select
the type. |
| Provider Rating | The rating that is assigned to the
service provider, such as (1) preferred, (2) approved, (3) not rated,
and (4) has issues. Click the Search icon and select
the rating. |
| Provider Type | The type of the service provider, such as primary, backup, and emergency. |
