Explanation
 An attempt to create a PLAN_TABLE failed. The
reason is that the CREATETAB authorization is missing. A formatted
print of the SQLCA and an error message are provided. Processing is
terminated.  
<V1> is the authorization ID.

<V2> is the SQL code.
