Create Incident application
You use the Create Incident application to report an incident or an event that has the potential to affect the health and safety of personnel, or to impact the environment. After an incident occurs, you can follow up with the persons who are affected to ensure that the appropriate corrective actions are implemented.
When you submit an incident record, it is available to other personnel to review. The Create Incident application works in combination with the Review Incident application.
For example, a maintenance technician who is working on an offshore oil platform needs to access a pressure transmitter to do routine maintenance. The transmitter is reached by using a passageway. On the passageway, the maintenance technician notices that a guardrail is badly corroded. The guardrail is not likely to provide sufficient protection if a person falls against it. The maintenance technician accesses the Create Incident application to report the hazard immediately to warn other personnel of the danger. The staff member who is responsible for the condition of the guardrail takes the appropriate action, and repairs the guardrail.
