List Failure Codes
Use the List Failure Codes dialog box in the Incidents application to report failure information for the asset or location on the record. To report failure information, you select failure codes that correspond with one or more levels of a failure hierarchy. The failure hierarchy describes the failure, the problem, the cause, and the remedy. You can create a hierarchy of one to four level. In other words, you can describe the failure only or the failure and the problem, and so on.
If you do not use the failure hierarchy to report information on the asset or location, you can use the Solution Details tab to enter free-form descriptions of the problem, cause, and remedy.
To report failure information:
- Display the Incident for which you want to report failure information. There must be a value in the Site field before you can report failure information.
- Click the Failure Reporting tab. If there is a failure class associated with the asset on the record, the system enters it in the Failure Class field.
- In the Failure Report table, click List Failure Codes.Note: If there is a value in the Failure Class field, the list of available failure codes is filtered by this value. To see all failure codes, clear the value in the Failure Class field.

Select one of the following choices:
To select only a top-level failure code and return to the record, click Select and Return .
To select a failure code and continue building a failure hierarchy, choose a failure code and click Select . The system displays the next level down in the failure hierarchy. Repeat these steps until you have entered the codes for each level of the hierarchy that you want.
To return to the incident with no selections, click Cancel .
- After you select these codes, you can enter a detailed account of the failure in the Remarks field and its associated long description field.
- Click the Save Incident icon.
