Use this section of the form to identify the service geographical areas that apply to this service provider.

| Find | Lists the available information. Click this action on the section bar and select from the listed geographical areas. Click OK. |
| Remove | Erases the information. Click this action. |
| Type | The type of the geographical area, such as city, country, county, and state. |
| Name | The name of the geographical area, such as Atlanta, Japan, Clark County, and Hawaii. |
| ID | The unique identifier of the geographical area. |
