Multipath failover causes kernel panic

In a multipath setup where SCSI disks are attached over multiple paths, failover might trigger a kernel panic.

For zfcp-attached SCSI disks, fast_io_fail_tmo should always be configured to 5 seconds and dev_loss_tmo should be disabled with value "infinity". This can be configured in the file /etc/multipath.conf.
