New monitor element for concurrently executing activities

A new monitor element, ACT_TOP, report the high watermark for executing activities so that you can better understand the peak query load on the database. The existing CONCURRENT_ACT_TOP element reports the high watermark for concurrent activities on the database in any state including executing or queued.
