Operation score cards

Within the debug.log of Accessers devices, or SlicestorOpera devices running Embedded Accessers, they print operation score cards on read or write failures. Typically, this is a result of failing to achieve a sufficient (threshold) number of consistent responses from Slicestors. In the past, each Slicestor could appear at most once in each score card. Now each Slicestor device can appear multiple times, as we see below:

Above, the same endpoint (IP and port) appears twice in the table. But each corresponds to a different Pillar (the first number in the right-most box followed by a colon).
