Goals data sets
The Goals folder contains data sets that you use to create reports relating to a client's goals.
Note: Goals is the primary data set in the Goals folder.
You must use data items from this data set when you join data sets in goal reports.
The Goals folder contains the following data sets:
- Goals
- This data set groups data items that relate to a client's goals such as the goal name, type, start date and end date. A goal is a measurable target that a client can achieve. Use this data set to identify a client's planned goals. If you want to display details of the users who added and completed the goals, use data from the Added By and Completed By folders.
- Progress Comments
- This data set groups data items that relate to comments recorded when progress is updated for a goal such as the comment, the name of the user who added the comment, and the date when the comment was added.
- Barriers
- This data set groups data items that relate to a client's barriers, such as the barrier name, category, and status. Barriers can hinder or prevent a client from achieving their goals or completing actions. Use this data set to identify barriers to a client achieving their goals. If you want to display details of the users who added and completed the barriers, use data from the Added By and Resolved By folders.
- Client Actions
- This data set groups data items that relate to a client's actions, such as the action name, reason, and outcome. Client actions are actions that clients can complete to help them to achieve a goal. If you want to display details of the users who added or completed the client actions, use data from the Added By and Completed By folders.
- Goals By Program
- This data set groups goals by their associated program. Use this data set to identify specific goals associated with a client's programs.
- Care Team Actions
- This data set groups data items that relate to actions care team members must complete to help a client achieve a goal, such as the action name, category, and status. If you want to display details of the users who added, are assigned to, and completed the care team actions, use data from the Added By, Assigned To, and Completed By folders.
- Barrier Goals
- This data set groups data items that relate to specific barriers associated with goals. Use this data set in reports to see a more granular view of specific barriers associated with a client's goals.
- Client Action Goals
- This data set groups client actions by their associated goals. Use this data set in reports to see a more granular view of specific client actions that are associated with a client's goals.
- Care Team Action Goals
- This data set groups care team actions by their associated goals. Use this data set in reports to see a more granular view of specific care team actions associated with a client's goals.
- Care Team
- This data set groups data items that relate to a client's care team members and their role in the care team. If you want to identify the care team member who added the goal, use this data set with the Users data set in reports. If additional roles are added for a care team member who is already on the client's team, their previous role is end dated and a new instance of that role is created with a new start date.
- Primary Care Team Member
- This data set groups data items that relate to members of the client's care team who are responsible for the client’s care plan and programs.
- Individual Demographics
- This data set groups data items that relate to client's demographic data such as their name, marital status, and gender. In addition, this data set groups a client's preferred or most recently updated address, phone number, and email address data.
- Individual Identification
- This data set groups data items that relate to client's identification types such as their Employee ID or passport, and the number associated with the identification type.
- Individual Tags
- This data set groups data items that relate to a client's tags, such as additional information that describes them like whether they are a member of an organization or group, for example, Student, Football Team.
- Individuals in Programs
- This data set groups data items that relate to a client’s programs such as the program name, program status, and the date and time from which a program is in a status.
