IKTINX2: I/O manager initialization

Write this routine if you want to use terminal input managers and terminal output managers you have written. IKTINX2 should perform the same function for user-coded input and output managers that module IKTIIOM performs for the IBM®-supplied I/O managers. IKTINX2 allocates storage for and initializes the I/O manager SRBs.
