Explanation
 The CONSOLE command attempted to establish a console
session with MVS™ console services,
but the attempt failed.  The MCSOPER macro, used to invoke MVS console services, returned the
indicated return code and reason code.

System action
 The CONSOLE command ends with a return code of
40.

an explanation of the MCSOPER return code and reason code.  Take the
action suggested in the explanation for the particular return code.
 If return_code from
MCSOPER is 4, after a CONSOLE ACTIVATE, then the console name is already
in use.  Use a different console name.
