Defining the structured RCA hierarchy
In the Investigations application, the administrator must define the hierarchy for your structured RCA before the investigator can enter the results of the analysis.
About this task
You need administrative privileges to define a structured RCA hierarchy.

Procedure
- In the Investigations application, on the List View tab, select the Add/Modify Structured RCA Hierarchy action.
- Specify the number of levels that you want to include in your hierarchy, and click Refresh.
- Provide a name for each level in your hierarchy.
- Optional: Upload an image that provides a visual representation of your
hierarchy. You can use the template that is attached to the following article to create your own image: Sample chart and data for a structured RCA hierarchy.
- Click Refresh Levels.
- Add or import your structured RCA data, and click OK.
- Close the Investigations application, and reopen it. The application must be reloaded before your hierarchy is available within the application.
- Select an investigation record, or create a new one.
- On the Analysis tab, open the Structured RCA subtab.
- In the Structured RCA Results section, click New Row.
- Verify that the new hierarchy is available for the investigator to use.
