Incidents overview
An incident record captures information about one or more events that deviate from an expected standard of operation.
The following examples are incidents with a health, safety, or
environmental impact:
- A fire extinguisher is missing from its designated location.
- The removal of a defective drain valve results in a substantial oil leak and requires activation of the environmental response team.
The removal of a defective drain valve is an example of a high-context incident. High-context incidents are narrative descriptions of important events that occur and the behavior of people in those situations. The documented behavior might be effective or ineffective and in general, is considered by the users to be critical. Analysis of high-context incidents can help to prevent future occurrence of the events and reinforce effective behavior.
