Creating consequence records
You create consequence records to describe and categorize the hazards that can occur in your work.
About this task
You create one consequence record for every combination of hazard consequence and hazard severity. For example, if your enterprise classifies consequences into seven categories and severity into five categories, you must create 35 consequence records.
For each consequence record, you create multiple risk likelihood records. Each record ranks the likelihood of the corresponding consequence and severity. For example, if your enterprise classifies likelihood into five categories, you must create five risk likelihood records for each consequence record. You assign a risk-based priority and a risk ranking to each risk likelihood record.
Procedure
- In the Risk Matrix application, click New Consequence.
- In the Consequence field, specify a unique identifier for the consequence.
- Complete more fields as needed.
- In the Risk List window, create one row for each likelihood that the consequence occurs. Assign a sequence number, a risk-based priority, and a risk ranking to each likelihood.
- Save the record.
