Creating surveys
A survey is a way of collecting information by evaluating the answers to formulated questions. Surveys are valuable for providing information about whether regulations are being followed during operational procedures or policies. You can use surveys to identify follow-up actions for improving the safety of the plant and its personnel. You can measure the level of agreement or of disagreement for each of the questions that are asked.
Before you begin
Develop the questions that you are going to ask during the survey. Depending on the goal of the survey, you can compile a number of different questions into sections. For example, for a climate survey, you can have a section of questions about attitudes toward safety. You can create a section of questions about compliance with procedures and regulations. You can create sections of questions that apply either to employees or to supervisors.
If you use a Likert scale in your questionnaire, you can add five responses to a question. Users who complete the survey can select the response that best describes their agreement or disagreement.
About this task
When you create a survey, you specify details about the context of the survey and the certifications that are essential. You also provide the questions for the survey, and how often it must be administered. By specifying the frequency of the survey, you can correlate surveys across different dates, and compare the results.
When you create a survey record, you can select the Survey Format option. When the Survey Format option is enabled, the numerical value that is assigned to the response for each question is hidden. You can enable the Survey Format option before you change the status of the record to Entry status.
Procedure
- In the People application, authorize personnel to approve surveys.
- In the Permit and Certificate Types application, define a permit or certificate type to associate with this survey record.
- If you are using a survey to test procedures, use the Operating Procedures application to create operating procedure records. You can also use an operating procedure to develop the steps that are essential for the survey.
- If you are evaluating the use of policies, use the Operating Policies application to create operating policy records or to reference an existing operating policy.
- In the Audit and Survey application, click New Audit and Survey on the toolbar.
- On the Audit and Survey tab, select Survey for the type of record.
- On the Questions tab, add and describe the questions in sequential order.
- On the Audit Team tab, specify the members of the team who are participating in the survey.
- Save the record.
