Scenario for preparing and administering a safety climate survey

Oil and gas workers in Company ABC often conduct safety climate surveys with the goal of improving health and safety. To assess the safety climate of an organization, Company ABC prepares a safety climate survey. Company ABC also develops a sample group and assigns the survey to the sample group.

Background

Company ABC plans to use Maximo® Health, Safety and Environment to collect information from the survey by evaluating the answers to the formulated questions. Company ABC uses the survey to specify actions as a follow-up to the survey. Company ABC also uses the survey to encourage employee participation in identifying safety challenges.

Patricia, the Survey Leader, is responsible for creating the survey and for assigning personnel to complete the survey. Fernando, the Survey Assessor, is responsible for analyzing the results of the responses from the sample group. Fernando is also responsible for defining the actions that are to be taken as a follow-up to the survey. Andrea is responsible for carrying out the actions that result from the survey.

Preparing the data

Several of the following steps describe the data that must be prepared before you specify the information for a safety climate survey. You might want to specify the following information ahead of time:
  • Assets by using the Assets application
  • Locations by using the Locations application
  • Operating procedures by using the Operating Procedures application
  • Operating policies by using the Operating Policies application
  • Permit and certificate type by using the Permit and Certificate Types application
  • Personnel by using the People application
  • Regulations by using the Regulatory Compliance application

The data is then available when you are ready to specify the fields for the safety climate survey.

Step 1 Develop the questions

Patricia develops the questions for the safety climate survey. Patricia compiles a number of questions into sections. One of the sections is called SAFETY. The SAFETY section includes questions about whether employees are using hearing protectors and safety glasses around the equipment in the boiler room. Patricia also develops the list of the employees to include in the survey, and assigns them to the MAINTENANCE sample group.

Step 2 Create the audit and survey records

Patricia uses the People application to authorize qualified, active personnel to approve the survey.

In the Audit and Survey application, Patricia creates an audit and survey template record SAFESUR with the type Survey. Patricia selects the Survey Format option. When the Survey Format option is enabled, the numerical value that Patricia assigns to the response for each question is hidden.

Patricia adds and describes each survey question in sequential order.

Patricia also specifies the members of the team who are to participate in the survey. This task is a nonmandatory step.

From the template record, Patricia creates the additional audit and survey records that are needed. Patricia also uses the Locations application to link the audit and survey records to the location of the boiler room.

In the Workflow Process application, Patricia creates a workflow process to be notified when each person completes the survey.

Step 3 Submit the surveys

In the Audit and Survey application, Patricia sets the audit and survey records to Survey Entry status. Patricia sends out an email to notify employees and to request their participation. When participants complete the survey, they set the survey status to Survey Submitted. The workflow process that is in place notifies Patricia of the completion of each survey.

Step 4 Analyze the responses and specify corrective follow-up actions

After the results of the survey are in, Fernando uses the Audit and Survey application to analyze the answers. To focus attention on findings that are critical to safety, Fernando reviews the answers to the survey questions. Fernando adds the findings to the survey. For example, Fernando determines that employees are generally not using safety glasses and hearing protectors in the boiler room. Fernando relates each finding, in sequential order, with the question that generated a response.

Fernando specifies a recommendation for each finding. Fernando suggests that a safety presentation is held. Fernando also suggests to provide training to employees who work in the boiler room. When a finding is associated with a function that is critical to safety, Fernando enables the Safety-critical checkbox.

As a follow-up to Fernando's recommendations, Fernando uses the Actions application to create and to approve an action that calls for safety presentations. Fernando also creates and approves an action that requests more safety training about protective gear. Fernando assigns Andrea to complete the actions.

Step 5 Complete the survey

After the safety presentation is held, and the safety training is provided, Andrea uses the Audit and Survey application to mark the actions as complete. Patricia changes the status of the survey to Complete. Patricia uses the work log to record observations about this survey.
