*Transmitter Name

Specifies the full name of the individual or organization that is transmitting the reports in this file. Left justify and space-fill the entry.

*Transmitter Address

Specifies the street address of the transmitter. Left justify and space-fill the entry.

*Transmitter City

Specifies the city of the transmitter. Left justify and space-fill the entry.

*Transmitter State

Specifies the state code of the transmitter if the Transmitter Country code is CA, MX, or US. Left justify and space-fill the entry. Space-fill if no entry or when Transmitter Country contains a code other than CA, MX, or US.

*Transmitter ZIP or Postal Code

Specifies the ZIP Code or foreign postal code for the transmitter. Do not include punctuation or formatting such as hyphens, periods, and spaces within the entry. An entry must be five or nine digits if the Transmitter Country is US. A nine-digit entry cannot end with four zeros or four nines if Transmitter Country is US. Left justify and space-fill this entry if less than nine digits.

*Transmitter Country

Specifies the transmitter country code.

*Transmitter Telephone Number

Specifies the transmitter telephone number. Enter all numbers as a single number string without formatting or punctuation such as spaces, hyphens, or parenthesis. Left justify and space-fill the entry.

*Transmitter Contact Name

Specifies the name of an official contact for the transmitter. Left justify and space-fill the entry.

*Transmitter Taxpayer Identification Number (TIN)

Specifies the parent financial institution’s EIN. If the financial institution does not have an EIN, enter the SSN of the institution’s principal owner. Do not enter hyphens, slashes, alpha characters, or invalid entries. Left justify and space-fill the entry.

*Transmitter Control Code (TCC)

Specifies the assigned eight-character Transmitter Control Code (TCC).

Transmitter User Field

Use this optional field for any descriptive information you may require; otherwise space-fill this field.

Transmitter Financial Institution

Enter the full legal name of the parent financial institution. Left justify and space-fill the entry.
