It has been a long day but I did not understand the bit "Note: if this disk hasn't been deallocated from the AIX LPAR, the path from this VIO server will report that it is down. "
Should that read "Note: if this disk HAS been deallocated (removed with rmdev) from the AIX LPAR, the path from this VIO server will report that it is down. "

Thanks, Nigel. What I meant was that if you remove the vscsi device from the VIOS without first removing the hdisk from the VIO client, the disk will be reported as missing on the AIX LPAR.

The correct order for removing a disk is:
1. Free up disk from any VG on the AIX LPAR (reducevg)
2. On the AIX LPAR: rmdev -dl hdiskN
3. On the VIOS: rmvdev -vtd vtscsiN

If you do step 3 without doing step 2 first, then the AIX error report will say that the path has gone missing. If it's the last good path, then on AIX the entire disk will go missing.
