Pinned topic DS3400 "Degraded wide port becomes failed"

Hi,

I was hoping someone could review a "All Support Data" dump from my DS3400 as I'm stumped on what the problem is.  Long story short the following critical alerts occurred

Event occurred: Apr 30, 2014 12:30:46 PM
Event Message: Optimal wide port becomes degraded
Event Priority: Critical
Component Type: Enclosure Component (ESM, GBIC/SFP, Power Supply, or Fan)
Component Location: Enclosure 0, Slot 1

Event occurred: Apr 30, 2014 12:30:46 PM
Event Message: Degraded wide port becomes failed
Event Priority: Critical
Component Type: Enclosure Component (ESM, GBIC/SFP, Power Supply, or Fan)
Component Location: Enclosure 0, Slot 1

Event Message: Logical Drive not on preferred path due to ADT/RDAC failover
Event Priority: Critical
Component Type: Controller
Component Location: Controller in slot A

I've tried the following to resolve the issue with no avail:

1. Redistribute Logical Drives
2. Update controller firmware (07.35.53.00 -> 07.35.73.00) and ESM firmware (1.93 -> 1.96)
3. Restart the Controllers
4. Reseat FC cables, replace FC cables
5. Reseat SFPs, replace SFPs
6. Reseat SAS cables in ESMs
7. Reboot ESX Host


Setup: DS3400 (1726-HC4) and 3x EXP3000 (1727-01X) connected to 2x Qlogic ISP2432-base 4GB FC HBAs on a standalone x3650 M2 (7947-62U) running VMware ESX 5.1 Update 1.
