Monica, great post! James Whittaker (of the former Google Test Director fame) mentions something similar in his book "How Google tests software". Google (and I am sure other test organizations) utilize Risk assessment tools to figure out frequency and impact of defects, and most certainly they let low grade defects go unfixed and they know it.
