Static text on SR long description

Hi All,
Is it possible to have a static text on SR long description? I would like to have few questions on SR long description so that when a SR is created the person who is entering the details will precisely know what information to provide.
I know this can be achieved through automation script, but I am trying to achieve this by modifying the application XML.
