Client referrals

Clients are manually registered in Watson Care Manager, or are automatically registered when they enter the application from an external system (via an API) or from one or more cohorts.

A cohort is a grouping of clients based on specific criteria such as age or at-risk priority, who are identified by an external system as requiring care management, and who are sent to the application by an electronic interface.

If organization units are enabled for your organization when clients are registered in IBM Watson Care Manager, they automatically subscribe to the principle of community consent where they agree to share their personal health information (PHI) in line with your organization's consent policies.

Active and Inactive Clients

By default, clients are registered in Care Manager as active clients. The care team might mark a client as inactive if the client is deceased, does not respond to care management, moved out of state, or declined to avail of further care. Occasionally a client might be marked as inactive in error.

If an inactive client is referred to the organization again from an external system via an API or cohort, the client is not automatically activated in the application. The supervisor must manually review each client referral to assess its validity before they activate the client or reject the client referral in the application.
