PM16676: Missing Add Policy button after deleting policy in LifeCycle

Error description

• In Rational Asset Manager 7202, when deleting policies, the
  button to add new policies disappear,
  and customer need to recreate the life cycle so the buttons show
  up  again.

Problem conclusion

• Rational Asset Manager now handles missing policies from
  lifecycles by showing a warning next to them.
  Once a policy governor is removed from the repository, all
  existing instances are void. Adding the governor back to the
  system will not fix these instances. A lifecycle manager
  will need to re-add/re-configure the policies.
  The existing/missing policies are left in place simply to
  show that you used to have one configured, but it is no
  longer available and so that you may take whatever action
  you need on this.
  Also handle transition conditions for missing policies and
  policy configurations and handle policy results for existing
  assets. otherwise, asset state history may fail to load
  correctly.
