  in Cognos 10.1.1 FP3  second cascading prompt,  list displays
  "all data" value in value list after any selection in first
  prompt
  in Cognos 10.2.2 any Fix Pack Version, after selecting any first
  value the second cascading value list changes and adds
  "ipfcfg_username", "----------------", then "all data", and
  other values.  The "ipfcfg_username" and "-----------"  should
  not appear, "all_data" should be top of list in second cascading
  value prompt
