Problem conclusion

• A customer has multiple xsd files and each one defines a
  namespace with a prefix called tns and each one has a unique url
  associated with it.  The code is currently ignoring a namespace
  if it has a prefix that has already been processed.  What the
  code should be doing is creating a unique prefix, such as tns1,
  tns2, etc.., if it processes a namespace with an existing
  prefix, but with a unique url.
