Error description

• Reproduced on Windows: Create a filemon control row monitoring
  c:\windows\system32\* on an interval of 1 second. Activate it
  and wait a few seconds, then delete the control row. After only
  a couple of attempts the SSM will usually crash.

  Upgrade to Fix Pack 8 for SSM 4.0.

Problem conclusion

• The thread pools used in filemon were being deleted before their
  jobs had completed. This is now impossible, and simplified: N*M
  pools have been replaced with 1 (where N control rows, M
  monitors). The simplification not only allows us to more easily
  ensure stability but should improve filemon's ability to scale
  without consuming as many resources.
  
  The fix for this APAR is contained in the following maintenance
